• Leaky or contaminated specimen

• Improper anticoagulant or preservative

• Improper storage or transport

• Improper/ unsafe collection container

  • e.g.- glass vacuum bottles for pleural fluids,
    fluids submitted in syringes, etc.

• Any lapse in Typenex procedure
