The Nutrition Clinic is located on the 8^th floor (Center) of the MCL Charity Campus at 1532 Tulane Avenue in New Orleans, Louisiana. Physicians may refer patients to the Nutrition Clinic by completing a nutrition clinic referral form. The referral form must be completed by the physician and may be faxed to the nutrition clinic where an appointment will be mailed to the patient. The patient may also hand deliver the completed referral form to the nutrition clinic to set up an appointment. It is imperative that the referral form is complete and legible and includes the patient’s name, medical record number, mailing address, and telephone number.
