The Department of Outpatient Diabetes Education is located on the 8^th floor (Center) of the MCL Charity Campus at 1532 Tulane Avenue in New Orleans, Louisiana. Physicians may refer patients to the Outpatient Diabetes Education department by completing the referral form. The completed referral form can be faxed, hand delivered, or called-in for appointment scheduling. Patients may be referred for self management classes (levels I, II, & III) and nutrition education. An individual appointment is scheduled with the dietitian once the patient has attended the Level I Self Management Class. After attending the Level I self management class, two follow up appointments will be scheduled with the dietitian.
